Job Description

Duties:

• Performs Patient assessments: Accurately assesses patient's appropriateness for hospice care, learning needs and progress toward goals. Develops the plan of care for the patient and family
• Ensures appropriate utilization of services: Maintains visit productivity standards. Demonstrates knowledge of insurance reimbursement .
• Completes discharge assessments: Performs laptop transfer of data twice daily. Completes admission assessment and transfers within 24 hours. Completes re-visit documentation and transfers
• Uses age appropriate teaching methods for patients and families: Evaluates teaching plan and revises as necessary. Documents patient education tools used Assumes responsibility for
• Case Management: Develops/implements/evaluates plan of care based on evidence-based knowledge.
• Revises care plan as indicated.Prioritizes care needs.
• Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines.
• Shows evidence of interdisciplinary communication.
• Performs HHA supervisions per standards.
• Ensures appropriate utilization of services.
• Follows scheduling process.
• Communicates progress toward goals with IDT.
• Attends and participates in IDT meetings every 2 weeks.
• Evaluates the plan of care and revises as needed
• Maintains safe environment for patients and co-workers.
• Follows infection control measures and reports infections.
• Demonstrates knowledge of pharmacology and safe medication management.
• Routinely reconciles medications.
• Maintains current medication orders
• Attends monthly staff meeting: Participates in PI meetings and activities/committees as requested.
• Accepts other assignments as requested.
• Assumes responsibility for case management: Prioritizes schedule to accommodate patient's needs.
• Documents case management activities.
• Demonstrates knowledge of pharmacology and safe medication management.
• Routinely reconciles medications.
• Maintain current medication orders.
• Participates in Performance Improvement Program: Reviews health records and gathers data to support PI activities.
• Attends UR meetings as requested.
• Provides orientation/ mentorship to new employees.
• Cross trains to learn new skills and demonstrates a willingness to teach other
• Assists with growing agency business: Provides orientation to new employees.
• Accepts other assignments as requested.
• Cross trains to learn new skills.

Skills:

• Must have 2-3 years as RN in home hospice environment
• Experience using an EMR
• Must have 3 years Hospice Care facility (not long term care)
• Must have Hospice Travel exp.
• Remote EPIC exp. strongly preferred
• Excellent critical thinking and communication skills
• Must have valid New Jersey driver's license
• BLS - required

Education:

• Current New Jersey RN License

Schedule Notes:

• On-call shifts required including overnights and weekends 1 -2 per month (4:PM and the second was 6:30:PM)
